Starting prompt (adapted)

Find the broken streetlights or other faults nearest my home on my utility company's repair page, including any asset or pole numbers the form asks for. File a repair request for each one and have replies sent to my email. Use only my name, address, and email, and ask me before sharing anything else. Tell me the reference for each request and check back later on its status.
